Do you or someone you know get help from the Consolidated Waiver (CW), Community Living Waiver (CLW), Person/Family Directed Supports Waiver (P/FDS), or the Adult Autism Waiver (AAW) from the PA Office of Developmental Programs (ODP)? If so, at some point, you/they will get a request to participate in IM4Q (Independent Monitoring For Quality) surveys. These surveys ask how well things are going for people who use these waivers. It’s a chance to share experiences and feedback.

Independent Monitoring for Quality (IM4Q)
The IM4Q Program has been active in Delaware County since 1999; one of the results of the Commonwealth’s Multi-year Plan for Pennsylvania’s Office of Developmental Programs. The Multi-year Plan called for the creation of local independent teams to monitor the quality of services and supports for people who have intellectual or developmental disabilities.
Focusing on individual consumer satisfaction, IM4Q is one of the few statewide programs of this kind in the country, pioneering community participation in the quality improvement process. Unlike DPW ‘licensure’ and Department of Health review and certification processes, IM4Q asks the person receiving supports and their family how they feel about the services they are receiving: what ‘works for them’ and what the consumer feels they need to improve the quality of their lives.

What is IM4Q?
IM4Q is an acronym that stands for Independent Monitoring for Quality. In the late 1990s, the state of Pennsylvania decided to implement a quality of life monitoring project to get feedback from service recipients and their families about their intellectual disabilities supports. By 2001, all counties in Pennsylvania were using a non-service providing entity to implement the standardized survey instrument.
Who is interviewed?
People registered with Delaware County OID may be randomly selected by PA’s Office of Developmental Programs. Participation is not mandatory. Individuals who have participated in the survey will be eligible after two years have passed.
What questions are asked?
Since IM4Q is a statewide initiative, the survey tool must be identical for each individual interviewed so that the aggregate data will be relevant for analysis. Questions are categorized into sections addressing satisfaction, dignity and respect, choice and control, inclusion, and physical setting. Answers to the first two sections of the survey may only be recorded if the individual is able to answer them. Other questions may be answered by a proxy (family member of support person).
Will IM4Q impact my services?
IM4Q survey participation will not impact services, however the individual and family may use the local considerations process to voice concerns that they would like to address with their Supports Coordinator.
